The incident occurred in/near UPPER SANDUSKY city. The TRUCK had been traveling in a northerly direction. The rail equipment was reported to have been traveling at 040 Mph with 3 locomotive(s) and 29 cars(s). It was cloudy, during the day and the temperature was 60. The railroad was operating on main line track over a public road crossing. The 31 year old male driver was stopped on the crossing and was reported to have stopped on crossing. There was 3 occupant(s) in the vehicle. The view of the track was not obstructed. There were 0 death(s) and 1 injuries in this incident. A Railroad Injury/Illness Report (form FRA-55a) was also filed. Hazardous material was not being transported by rail or highway vehicle. The crossing was protected by: Crossbucks WILI50 TRAVELING EAST ON FORT WAYNE LINE WHEN TRUCK FAILED TO YIELD AT RAIL ROAD CROSSING, AND STOPPED WITH FRONT OF VEHICLE FOULED SINGLE MAIN AND WAS STRUCK BY CR8213. DRIVER AND PASSENGER TAKEN TO HOSPITAL, TREATED AND REL EASED WITH CUTS/ BRUISES. DRI ER TO BE CITED FOR FAILURE TO YIELD.
